Case History : Herniated lumbar disc with sciatica, pain and numbness

PS is a 30 year old housewife and mother who had experienced recurring episodes of low back pain for the past 10 years. This pain was progressively more severe for the past two years. A month before admission to the hospital she awoke with excruciating low back pain, and pain with numbness down the back of her right leg. She complained of weakness in the leg and the weakness progressed although the pain had improved.

Examination demonstrated pain when the right leg was passively elevated. There was weakness in the muscles which flexed up the foot at the ankle, numbness to pin prick over the top of the foot. The reflexes at the knee and ankle were normal.

A MRI image demonstrated a very large herniated disc fragment compressing the right L5 nerve root. A microlumbar discectomy was performed because of the failure of conservative treatment and the presence of severe pain and weakness. She left the hospital the same day and enjoyed immediate, near complete relief of sciatic pain.

The weakness is greatly improved, but the numbness persists. Comment: The L5 nerve root is usually the root compressed by a fragment of disc material which prolapses from the disc space between the 4th and 5th lumbar vertebrae. The nerve root controls the muscles of elevation of the foot at the ankle, elevation of the big toe, sensation on the top of the foot…

Micro Discectomy – What is it ?

Through advancements in modern surgical technology surgeons worldwide have started performing a surgical procedure called a micro endoscopic discectomy (MED). The operating surgeon uses special instrumentation combined with a real-time camera display to remove a herniated disc or fragments of one through a very small incision.

A microscopic discectomy is performed for the same reasons as a traditional discectomy to accomplish the same results. An endoscopic microdiscectomy is performed to remove a herniated disc by using much smaller incisions. In a traditional discectomy your surgeon makes a large enough incision which he can visually see and remove herniated disc fragments. Overview

 

MicroEndoscopic Discectomy (MED)

A patient is brought into the operating room and is put under general anesthesia. Some surgeons have chosen to perform MED under local or spinal anesthesia allowing the patient to stay awake throughout the procedure. The patient is turned onto his abdomen and padded into position. A fluoroscope (floor-o-scope, a machine which projects live x-ray pictures onto a screen) is brought in for use during the remainder of the operation. The patient’s back is scrubbed with sterile soap, and a sterile field is created. Drapes are placed accordingly, and the surgery begins…

 

The disc space is confirmed using the fluoroscope, and a long acting, local anesthetic is injected through the muscle and around the bone protecting the disc. A half to one-inch incision is made. A thin wire is placed through the incision and lowered until it touches the bone. Progressively larger dilators are brought down on top of one another following the wire. In this manner, the muscle is stretched rather than cut. By the time the 4th or 5th dilator is placed, the muscles are stretched to an opening roughly the size of a nickel.

 

It is through this opening that the procedure is performed. Over the last dilator, a working channel is positioned; this circular retractor holds back the muscles and now the dilators can be removed. The retractor is held in place by a mechanical arm attached to the table…

  • Swelling of head, neck, and arms as a result of pressure on large blood vessels
  • Heart failure – an ascending aneurysm may affect the heart valves, causing blood to back up into the heart.

How is a thoracic aortic aneurysm diagnosed ?

In addition to a complete medical history and physical examination, diagnostic procedures for a thoracic aortic aneurysm may include any, or a combination,of the following :

Computed tomography scan (Also called a CT or CAT scan) : – A CT scan shows detailed images of any part of the body, including the bones, muscles, fat, and organs. CT scans are more detailed than standard x-rays.

Magnetic resonance imaging (MRI) : – a diagnostic procedure that uses a combination of large magnets, radiofrequencies, and a computer to produce detailed images of organs and structures within the body.

Echocardiogram (ECG/EKG) : – a procedure that evaluates the structure and function of the heart by using sound waves recorded on an electronic sensor

Transesophageal echocardiogram (TEE) : – a diagnostic procedure that uses echocardiography to assess the presence of an aneurysm, the condition of heart valves, and/or presence of a dissection (tear) of the lining of the aorta. TEE is performed by inserting a probe with a transducer on the end down the throat.

Chest X-ray : – a diagnostic test which uses invisible electromagnetic energy beams to produce images of internal tissues, bones, and organs onto film.

Arteriogram (angiogram) : – an x-ray image of the blood vessels used to evaluate various conditions, such as aneurysm, stenosis (narrowing of the blood vessel), or blockages…

 

Treatment may include :

Routine MRI or CT : – to monitor the size and rate of growth of the aneurysm.

Controlling or modifying risk factors : – steps such as quitting smoking, controlling blood sugar if diabetic, losing weight if overweight or obese, and controlling dietary fat intake may help to control the progression of the aneurysm.

In the lumbar spine, the spinal canal usually has more than enough room for the spinal nerves. The canal is normally 17 to 18 millimeters around, slightly smaller than a penny.
